High Blood Pressure |
Goal
- Reduce risk of heart disease, stroke, and other complications of High Blood Pressure.
Target
- Eighteen years of age and older: BP < 140/90.
- Diabetes or Kidney Disease: BP < 130/80.
Care Plan
- 30—60 minutes exercise every day.
- Follow the DASH Diet.
- Medications as advised by your physician.
- Take and record your blood pressure 2-3 times per week or as recommended.
- Labs: CBC, BMP, U/A before your office visit or as directed.
- Office visit:
- every 6 months if blood pressures meet target.
- As soon as possible if blood pressures do not meet target.
Tools needed
- Blood pressure cuff.
- Blood pressure diary.
- DASH eating Plan.
- Comfortable walking shoes.
- Medications as advised.
