High Blood Pressure

Goal

  • Reduce risk of heart disease, stroke, and other complications of High Blood Pressure.

Target

  • Eighteen years of age and older: BP < 140/90.
  • Diabetes or Kidney Disease: BP < 130/80.

Care Plan

  • 30—60 minutes exercise every day.
  • Follow the DASH Diet.
  • Medications as advised by your physician.
  • Take and record your blood pressure 2-3 times per week or as recommended.
  • Labs: CBC, BMP, U/A before your office visit or as directed.
  • Office visit:
    • every 6 months if blood pressures meet target.
    • As soon as possible if blood pressures do not meet target.

Tools needed

  • Blood pressure cuff.
  • Blood pressure diary.
  • DASH eating Plan.
  • Comfortable walking shoes.
  • Medications as advised.
