Diabetes Mellitus

Goal

  • Prevent the complications of Diabetes including stroke, heart attack, kidney failure, blindness, nerve damage, and gangrene.

Target

Tools needed

  • Blood sugar monitor.
  • Blood sugar diary.
  • Healthy eating plan.
  • Comfortable walking shoes.
  • Medications as advised.

Care Plan

  • 30—60 minutes exercise every day.
  • Follow a healthy diet. Here are two resources: DASH Diet, American Diabetes Association.
  • Medications as advised by your physician.
  • Record your blood sugar 2-3 times per week or as recommended.
  • Labs: HbA1c, Fasting BMP, lipids before your office visit or as directed.
  • Office visit:
    • every 3 months if blood sugars meet target.
    • As soon as possible if blood sugars do not meet target.
