Tools needed
- Blood sugar monitor.
- Blood sugar diary.
- Healthy eating plan.
- Comfortable walking shoes.
- Medications as advised.
Care Plan
- 30—60 minutes exercise every day.
- Follow a healthy diet. Here are two resources: DASH Diet, American Diabetes Association.
- Medications as advised by your physician.
- Record your blood sugar 2-3 times per week or as recommended.
- Labs: HbA1c, Fasting BMP, lipids before your office visit or as directed.
- Office visit:
- every 3 months if blood sugars meet target.
- As soon as possible if blood sugars do not meet target.
